What to expect in the hours, days and weeks after hyaluronic acid penile filler, based on Hardrock Clinic’s aftercare protocol. Your own written instructions always take priority.

Short answer
Most men return to normal daily activities the same day. Swelling, tenderness, mild bruising and temporary asymmetry are common at first and begin to settle over the first week. The clinic recommends approximately 7–10 days without sexual activity, followed by a physician-directed return, and the final contour develops over the following weeks. Advice on exercise and flying is given individually at discharge.
Recovery
First 24 hours
Seek urgent medical care if you have severe or increasing pain; skin that turns pale, blue, dusky or dark; blistering; fever with spreading redness; inability to pass urine; or a painful erection lasting several hours. Also contact the clinic on your discharge number. The risks and safety page explains these signs.
The procedure is performed under local anaesthesia and you go home the same day with a light dressing, written aftercare instructions and a direct contact number. Keep that number with you for the whole of your recovery, including after you travel home.
In the first day, swelling, tenderness, temporary asymmetry and mild bruising are common. The penis will look larger than the settled result because of swelling, so this is not the time to judge the outcome. Uneven swelling can also make one side look fuller; symmetry is assessed later, once swelling has gone down.

Recovery
Days 2–7
Swelling begins to settle during the first week. Hygiene is straightforward: follow the written instructions you were given about the dressing and washing.
- Activity: avoid strenuous physical activity during this period. Ordinary daily activities are usually fine.
- Sexual activity: none. The abstinence period continues into the second week.
- Massage: only if and as the clinic instructs. Do not massage or manipulate the area on your own initiative.
- Bruising and tenderness: these usually fade over this time. Pain that is getting worse rather than better is not expected — contact the clinic.
If you notice a lump, persistent unevenness or swelling that seems to be moving towards the foreskin, report it to the clinic rather than waiting. Uncircumcised men should read the guide for uncircumcised men, because foreskin swelling and migration need particular attention.
Recovery
Week 2
The clinic recommends approximately 7–10 days of sexual abstinence after treatment. For most men that period ends during the second week. The return to sexual activity is physician-directed: it depends on how your tissues have settled, so confirm with the clinic before resuming rather than counting days alone.
Swelling continues to settle, but what you see is not yet the final result: the contour keeps developing over the following weeks, and any remaining unevenness is assessed then.

Recovery
Weeks 3–6
Over the following weeks the final contour develops. This is the point at which symmetry is assessed and at which it makes sense to judge the result. How the result is measured, and how published figures compare with the clinic’s own, is explained on the penile filler results page.
Follow-up is arranged if required. The Advanced 15 cc package includes a follow-up control, and the Premium 20 cc package includes follow-up and a touch-up according to clinic policy and physician assessment. Correction, top-up or dissolution outside a package is priced case by case.
Recovery
Long-term
Hyaluronic acid is gradually resorbed by the body, so the result slowly reduces over time. Published follow-up most often covers around 12–18 months, with wide individual variation. The guide to how long penile filler lasts sets out the data.
When the result has faded, some men discuss a top-up with the physician. Whether it is appropriate, and how much gel would be used, is decided after examination.
Tell any doctor who treats you in future that you have had hyaluronic acid filler, particularly before any procedure or examination involving the genital area.

Exercise, flying and long-haul travel
The clinic protocol does not set a fixed number of days for these. The physician gives personalised advice at discharge, based on your treatment and how you are recovering. Ask about your specific plans, including the date of your flight home.
Follow-up and contact
Follow-up is arranged if required. Keep the direct contact number from your discharge instructions, and report anything that is not settling as expected.

Will I need time off work?

When can I have sex again?
The clinic recommends approximately 7–10 days of sexual abstinence, followed by a return to sexual activity when the physician advises.
When can I fly home?
There is no fixed day count in the clinic protocol. The physician gives personalised advice about flying and long-haul travel at discharge.
Should I massage the area?
Only if and as the clinic instructs. Do not massage or manipulate the filler on your own initiative.
When will I see the final result?
Early swelling makes the penis look larger than the settled result. The final contour develops over the weeks after treatment.
