Published studies report follow-up of roughly 12–18 months, and one large audit reported a mean longevity of 12 months with a range of 9–24 months. How long it lasts in any one man depends on his metabolism, the product, the volume and how much mechanical stress the area receives. Individual results vary, and no one can promise a permanent result.

Why hyaluronic acid filler does not last for ever
Hyaluronic acid occurs naturally in the body, and the body has its own enzymes for breaking it down. Unmodified hyaluronic acid disappears within days. Filler gels are cross-linked so that they resist this breakdown for months, but they are still biodegradable: the gel is slowly resorbed and the volume it provides reduces over time.
This is a deliberate trade-off. The same property that makes the result temporary is what allows the gel to be dissolved with hyaluronidase if there is a problem — something that permanent materials such as PMMA do not allow. Resorption is usually gradual rather than sudden. Most men notice a slow softening and reduction in fullness rather than a single point at which the result disappears.

Two points are worth reading carefully. First, a study that followed men for 18 months tells you that some effect was still measurable at 18 months — it does not tell you what happened afterwards, because follow-up stopped. Second, the manufacturer’s lifetime estimate describes the face, where the tissue, movement and injection plane are different. It is useful context but not a penile figure. What affects how long it lasts
The product
Fillers differ in concentration and degree of cross-linking, which affects how firmly the gel holds together and how quickly it is resorbed. Firmer, deep-volume gels are generally chosen for this use.
Volume

Mechanical stress and lifestyle
The penis is a mobile area subject to friction and changes in size. Published data do not rank specific activities or lifestyle factors, so follow the clinic’s written aftercare instructions.

Results
What happens to the filler after two years?
The honest answer is that the published evidence becomes thin at this point. The longest penile follow-up in the sources cited here is around 18 months, and the highest individual longevity reported in the 324-patient audit was 24 months. Beyond two years there is not enough published penile data to give reliable figures.
What can be said is that hyaluronic acid is biodegradable, so the expectation is that the added volume continues to reduce and the penis returns towards its pre-treatment size. Some men may notice a small residual fullness for longer than others; a published report of four cases describes residual volumes after penile hyaluronic acid filler that were corrected with hyaluronidase. You should not plan on any part of the result being permanent.

Repeat treatment and top-ups
Because the result fades, men who want to maintain it usually consider a repeat treatment or top-up. Whether a top-up is appropriate, when, and with how much gel, is decided by the physician after examining you, because the amount of filler remaining differs from man to man.

Setting realistic expectations
It helps to think of hyaluronic acid filler as a result that lasts for a period measured in months, most often around a year to eighteen months in published reports, with wide variation either side. Men who want a permanent change should understand that hyaluronic acid will not provide one, and that permanent injectable materials carry their own, harder-to-reverse risks.
