Non-Surgical Penis Enlargement with Hyaluronic Acid Filler

Hyaluronic acid filler is the main non-surgical, medically supervised way to increase penile girth. This page explains what it changes, what it does not, and how it compares with the other things marketed as enlargement.

The short answer

Short answer

Hyaluronic acid filler can increase penile girth without surgery, under local anaesthesia, and the effect is temporary and usually reversible. It does not make the penis longer and is not a substitute for surgical lengthening. Published studies most often report a mean flaccid girth gain of around 2–2.5 cm, with individual variation; pills and creams lack evidence, and injections of non-medical substances cause serious harm.

Treatment

Girth, not length

The most important thing to understand about filler is what it changes. The gel is placed in the loose dartos layer around the shaft, outside the erectile bodies and the urethra. It adds circumference. It does not lengthen the erect penis, it does not change erectile function, and it is not a treatment for erectile dysfunction or curvature.

Some men find that a thicker shaft looks different in proportion, but that is a change in appearance, not in length. If length is your main concern, filler is not equivalent to surgical lengthening and should not be chosen as a substitute for it. Treatment

How hyaluronic acid filler is performed

At Hardrock Clinic the treatment takes approximately 30–45 minutes under local anaesthesia, and the gel itself contains lidocaine. No general anaesthesia or sedation is used. The gel is delivered in small amounts through several entry points using a fan technique, then moulded and checked for symmetry. Patients go home the same day with a light dressing, written aftercare and a direct contact number, and most men return to normal daily activities the same day.

The clinic uses Teosyal Ultra Deep in volumes of 10, 15 or 20 ml. Its approved indications are facial, so penile use is off-label. The full technique is described in the procedure section of the hyaluronic acid guide.

What results to expect

Published evidence

A randomised trial reported a mean girth gain of 16.95 mm at 48 weeks with hyaluronic acid. A 2025 audit of 324 men (conference abstract) reported a mean flaccid gain of 2.5 cm, range 1.5–4 cm, at a mean volume of 15 ml, with mean longevity of 12 months.

Clinic-reported figures

Hardrock Clinic reports approximately +4 cm, +6 cm and +8 cm circumference from 10, 15 and 20 ml. These come from the clinic’s own caseload, sit above published means and are not guaranteed.

The result is temporary because the body gradually breaks the gel down, and it varies between individuals. A 2021 systematic review of 57 studies of penile enhancement procedures concluded that the quality of evidence was low. For more detail on reading these figures, see penile filler results.

Treatment

The danger of non-medical injections

Some men try to enlarge the penis by injecting, or having someone inject, substances that are not medical fillers: paraffin, mineral or vegetable oils, petroleum jelly, silicone and similar materials. The medical literature on this is consistent and serious.

  • A 2021 review of sclerosing lipogranuloma of the penis describes the inflammatory reaction these foreign substances provoke, the complications that follow and their treatment, which is often surgical.
  • A 2019 review of penile paraffinoma describes the same problem arising from paraffin injection.

These materials cannot be dissolved. When they cause problems, correction usually means surgery. If you have had any such injection in the past, tell the physician: previous penile procedures are part of the assessment and may affect whether filler is appropriate.

Treatment

Is enlargement the right answer?

Many men who seek enlargement are within the normal range. A nomogram built from measurements of more than 9,000 men found a mean flaccid circumference of 9.31 cm and a mean erect circumference of 11.66 cm.

European Association of Urology guidelines recommend psychological evaluation for men of normal size who seek penile augmentation, and professional societies in North America and internationally have published cautious statements on cosmetic penile procedures. In line with that, the clinic may advise against treatment when the concern appears body-dysmorphic in character or when expectations cannot be met. Those are not obstacles for their own sake; they protect men from a procedure that will not resolve the underlying worry.

Common questions

Can filler make my penis longer?

No. Hyaluronic acid filler adds girth only. It does not lengthen the erect penis and is not equivalent to surgical lengthening.

Do enlargement pills or creams work?

There is no reliable evidence that pills, supplements or creams increase penile size.

Does the clinic offer traction devices?

No. Traction and vacuum devices are outside the scope of this clinic’s programme, and this site does not assess them.

Is filler permanent?

No. The gel is gradually broken down, and it can usually be dissolved with hyaluronidase if needed.
