01 — The treatment
What is penile filler?
Short answer
Penile filler is the injection of hyaluronic acid gel into the loose subcutaneous layer of the penile shaft to increase its circumference. It is non-surgical, performed under local anaesthesia in roughly 30 to 45 minutes, involves no incision and no surgical scar, and is temporary — the gel is gradually broken down by the body.
The material used is the same class of cross-linked hyaluronic acid gel used in facial aesthetic medicine. Hyaluronic acid is a naturally occurring glycosaminoglycan found throughout human connective tissue; the injectable form is chemically cross-linked so that it holds its shape and resists rapid breakdown.
The gel is placed in the plane between the skin and Buck fascia — the loose, mobile dartos layer. That specific plane matters. It is superficial to the erectile bodies, so the corpora cavernosa, the urethra and the neurovascular bundle are not entered. It is also mobile enough to allow the gel to be moulded and distributed evenly around the shaft after injection.
What the treatment changes is girth. It does not lengthen the erect penis, it does not alter erectile function, and it is not a treatment for erectile dysfunction or for curvature. Those are separate problems with separate answers, and a filler is the wrong tool for them.

Distributing small amounts across many entry points, rather than a few large boluses, is what keeps the contour even — and it is the same reason the marks in the diagram sit at equal intervals around the shaft.
02 — Motivation
Why men choose penile girth enhancement
Most men who enquire are not in the range that medicine would call abnormal. They are in the normal range and dissatisfied with it — and that dissatisfaction is a legitimate thing to discuss with a urologist rather than something to be dismissed.
A change after another procedure or condition
Some men notice reduced girth after prostate surgery, Peyronie disease, or a penile implant, where the shaft can feel narrower than before.
Wanting a reversible option first
Hyaluronic acid is temporary and can usually be dissolved. For men unwilling to accept a permanent surgical change, that is often the deciding factor.
Avoiding surgery and its downtime
No incision, no graft, no general anaesthesia, and most men return to normal daily activities the same day — subject to their physician’s advice.

Where the underlying concern is body dysmorphic in character, or where expectations are fixed on a measurement that filler cannot deliver, injecting is not the right answer. Part of the consultation is establishing that. Some men are told that no volume of filler will produce what they are describing, and that is a legitimate outcome of an honest assessment.

03 — The material
Why hyaluronic acid?
Short answer
Hyaluronic acid is preferred for penile girth work because it can be dissolved. Hyaluronidase breaks it down, which means an unsatisfactory result, a nodule or an over-filled area can usually be reversed — an option that does not exist with permanent materials such as silicone or PMMA.
The urological literature on penile enhancement complications is, to a large extent, a literature about permanent and non-medical materials. Reviews of penile enhancement complications consistently note that hyaluronic acid, used with standardised low-volume technique, carries a lower complication burden than polylactic acid, PMMA, silicone or self-injected non-medical substances — and unlike those materials, its complications have a medical rather than a surgical answer.
The randomised evidence base compares hyaluronic acid against polylactic acid. In an 18-month randomised trial of 67 men, girth remained significantly above baseline in both groups at 18 months, with no significant difference between the two materials and significantly improved satisfaction in both. Hyaluronic acid therefore has no demonstrated durability disadvantage against its main injectable competitor, while retaining reversibility.

Why Teosyal Ultra Deep?
Short answer
Teosyal Ultra Deep is a CE-marked hyaluronic acid filler made by Teoxane Laboratories in Geneva, Switzerland. It is a high-G′, high-cohesivity gel designed for deep placement and volume support rather than fine-line correction, which is the rheological behaviour wanted when the goal is to hold volume in a mobile subcutaneous plane.
The clinic uses it because those handling characteristics suit the anatomy: a gel that resists deformation holds a distributed contour better in tissue that moves and stretches, and the manufacturer’s quality and traceability standards are those of a mainstream European device manufacturer.
We have not published a claim that this product is clinically proven for penile use, has superior longevity in the penis, or stimulates lasting collagen there, because we could not find evidence specific to that application.
Off-label use — stated plainly
Teosyal Ultra Deep is CE-marked for deep facial volumisation. It does not carry a specific approved indication for injection into the penis. Its use for penile girth enhancement is therefore an off-label application of a licensed medical device — a recognised and lawful practice in which a physician takes personal clinical responsibility for using an approved product outside its labelled anatomical indication.
This is true of essentially every hyaluronic acid filler used for penile augmentation worldwide, not only of this product.

How penile filler is performed
Short answer

Why does 20 cc not simply mean twice the gain of 10 cc? Because circumference is a function of area, and filler is spread over the surface of the shaft. The same volume distributed over a longer or already-wider shaft produces a smaller measured circumference change. Two men receiving identical volumes can measure very differently.
The plate above is drawn from that arithmetic rather than around it. Circumference is what the treatment changes, so the geometry works backwards from the circumference figure: added girth widens the radius by that figure divided by 2π, which is what thickens the layer you can see between the skin and Buck fascia. It is also why the two structures inside that layer — the corpora cavernosa and the urethra — are exactly the same size in all four states. Nothing is being added to them.

Short answer
Volume is visible immediately, but the honest measurement is taken later. Published series of hyaluronic acid penile girth enhancement most often report a mean flaccid girth increase of around 2 to 2.5 cm. In a 2025 audit of 324 patients treated with a mean of 15 mL, the mean increase in flaccid girth was 2.5 cm, with individual results ranging from 1.5 cm to 4 cm.

Those four figures all come from the same retrospective single-centre audit of 324 patients published in The Journal of Sexual Medicine in 2025. They are not this clinic’s figures, and they are not a promise — they are the best available indication of what this treatment typically achieves in practice.
We publish them because they are more useful to you than a marketing number, and because they set the frame for reading any clinic’s claims — including ours. The clinic’s own reported figures — approximately +4 cm at 10 cc, +6 cm at 15 cc and +8 cm at 20 cc — sit above what published literature currently supports, and we say so directly rather than quietly. How long does penile filler last?
Short answer
Penile filler results commonly remain visible for around 12–18 months, although longevity varies according to filler volume, metabolism, anatomy and individual tissue response.
The 324-patient audit reported a mean filler longevity of 12 months with a range of 9 to 24 months. The 18-month randomised trial found girth still significantly above baseline at 18 months. So the 12–18 month figure is a reasonable expectation, with genuine variation on both sides of it.

On lasting collagen effects: some tissue remodelling may occur following hyaluronic acid treatment, but the amount of long-term residual enhancement varies and should not be considered guaranteed. We do not publish a specific percentage of permanent retention, because we found no clinical literature supporting such a figure for penile application. Nobody should promise you permanent enlargement from a resorbable filler.
10 — Recovery
Penile filler recovery
Short answer
Most men return to normal daily activities the same day. Swelling and tenderness are expected for the first few days, sexual abstinence of approximately 7–10 days is recommended, and the final contour settles over the following weeks.
First 24 hours
Swelling, tenderness, temporary asymmetry and mild bruising are common. Volume looks larger than the settled result.
Days 2–7
Swelling begins to settle, the gel integrates, hygiene is straightforward, and strenuous physical activity is avoided.
Days 7–10
Physician-directed return to sexual activity. Approximately 7–10 days of abstinence is the clinic’s recommendation.

11 — Safety
Risks and safety
Short answer
Penile filler is not risk-free. Published series report predominantly mild and manageable events, and no clinic can promise a complication-free outcome. What a good clinic can offer is technique that lowers risk, honest disclosure of what can go wrong, and a plan for managing it.
In the 324-patient audit, reported complications were: migration of filler into the foreskin in 7.7%, asymmetry in 6.1%, filler lumps in 4.6%, infection in 1.5% and requested dissolution in 1.2%. All were managed — with hyaluronidase, additional filler, or oral antibiotics — and no serious adverse events were reported. A separate series of 155 men found 12.3% required cosmetic correction.
Those numbers are worth reading twice, in both directions. They show a treatment whose complications are usually correctable rather than catastrophic. They also show that a meaningful minority of men need something done afterwards, which is not the impression most clinic websites give.

These are patient statements held by the clinic and published with permission, shortened for length and identified by first name and country only — this is a private procedure and the attribution is the part a patient would be recognised by.
None of them quotes a measurement. Girth figures appear on this site only where the study they come from can be named, and a personal account is not that kind of evidence. Read them for what the process is like, not for what it will do.
I sent the same five questions to three clinics. This was the only one that came back with figures and a source instead of promises, and that is ultimately why I got on the plane.
Discretion was what I cared about most. The procedure was never named out loud at the desk, and the lounge is seating in separated pairs instead of a row of men avoiding each other.
The coordinator dealt with the hotel, the transfer and my questions at two in the morning in my own time zone. I never had to explain my situation twice to a different person.
What decided it was being told, without asking, what would be done if I disliked the result — that hyaluronic acid can usually be dissolved again. Nobody else volunteered that.
I asked to see photographs of other patients and was politely refused. Strangely, that is the moment I started trusting the place — my own privacy is worth the same.
Twenty-four hours of flying for a forty-minute procedure sounds absurd. Having read what the surgical alternatives involve, it was the right absurdity.
It is a proper medical floor with a consultation room and a door that closes, not a rented room above a shop. That mattered more to me once I was standing in it.
Confidence in a changing room is a small thing that turns out not to be small at all. The procedure itself was the least dramatic part of the year.
The consultation talked me down from the volume I had asked about. Being told what my anatomy would carry, rather than being sold the largest package, is not what I expected.
Local anaesthetic, about forty minutes, and I walked out and had dinner. The first week of swelling looked alarming until I re-read the aftercare sheet, which had said exactly that.
The doctor is a urologist before he is anything else. He examined me properly and asked about medication and history, and at no point did it feel like an aesthetics shop.
The cash price on the website was the price at the desk. No consultation fee appeared, no deposit was asked for and nothing was added on the day.
My measurement was taken the same way at the consultation and again at follow-up, so the comparison actually means something rather than being a feeling.
Two small lumps appeared in the second month. I messaged, I was seen on video the same week, and they were dealt with without fuss and without an invoice.
A year later it is still visibly there — less than at week two, which is precisely what I was told would happen. Expectations set properly are worth a lot.
The anaesthetic was uncomfortable for about a minute. After that there was pressure and nothing else, and I spent most of it on my phone.
Everything was explained twice — once on WhatsApp before I travelled and again in the room before anything was done. Nothing on the day was a surprise.
I was told at the outset that the first week is not the result and that the contour settles over the following weeks. That turned out to be the single most useful sentence anyone said to me.
In Thursday, treated Friday, home Sunday. What I did not expect was that the follow-up messages kept arriving for weeks after I was back and nobody was selling me anything.
Bruising and tenderness for a few days, back in the gym after a week as instructed, and ten days of abstinence, which was easily the hardest part of the whole thing.
Arabic-speaking coordination from the first message to the last. My questions and my wife’s questions were answered with the same patience.
Nobody promised me permanence. I was given twelve to eighteen months and told to budget for a repeat if I wanted to keep it, which is the honest answer.
I arrived with ten questions written down and left with ten answers, including the two about risks that the other clinics I contacted had quietly skipped.
